Chronic Disease Prevention and Management

At Unison Health and Community Services, we believe that individuals should take control of their health and play an active role in their own health care. We offer a number of programs that help prevent and manage various chronic health conditions, including diabetes, HIV and Hepatitis C. Our staff will provide extensive information about the diseases. You can learn about lifestyle changes that can help stop the onset of a chronic condition and manage the existing ones. We will let you know about the resources available to you both at Unison and in the community.

Take Charge

Available at:

Take Charge is a 6-week chronic disease self-management workshop for people with heart disease, arthritis, diabetes, asthma, bronchitis, emphysema and other conditions. It helps you build skills to cope with your chronic condition; offer opportunities to share your experience with other group participants and support each other.
The workshop is offered in partnership with North York General Hospital four times a year.
